MAKING A BEQUEST THROUGH A WILL:

Since Wills and Trusts are legal documents, you should seek an attorney’s help to assist with this process. Each individual’s situation is unique. Suggestions for Leaving a Specific or Proportionate Bequest:

I, (name), of (city, state, ZIP), give, devise and bequeath to the ACPA Foundation a not-for-profit organization whose Federal Tax Identification Number is 52-1892948, (written $ amount or percentage) of my estate, or property described herein (or the remainder of my estate) as an unrestricted gift to be used at the discretion of the ACPA Foundation Board of Trustees as they deem in the best interest of the Foundation.

• Complete the Confidential ACPA Foundation Giving Form. If appropriate, include a copy of the relevant portion of your will or trust document
